Save time at check-in: complete your intake forms online before your visit. Please also bring a photo ID, your insurance card and any recent imaging (X-ray or MRI reports from the last two years, if you have them).
- New patient intake form — health history, symptoms and goals (10 minutes online).

We Accept Your PPO Plan
We work with all PPO insurance plans and most major carriers. Send us your details before your visit and we'll verify your benefits free of charge, so you know your coverage before you arrive.
No insurance? No problem. Ask about our transparent self-pay rates and the $89 new patient special.

What to Expect

Welcome & conversation (10 min). We review your forms and hear your story in your own words.

Hands-on examination (15 min). Posture, movement and joint testing to find the true source.

First treatment (15 min). When appropriate, your first gentle adjustment happens the same day.

Your roadmap (5 min). A clear plan: visits, milestones and home exercises.
